Crown failures

Several have been mentioned following tuning=20
I remember Hitchin failing in this way.
How many times has this happened?
Please could I have any known details with dates to compile a list, or perh=
aps one exists.
These can be sudden catastrophic fails.
Has there been any injury or serious damage as a result?
