Hitchin tenor gudgeon broke... list not starting too well??? !

Crown failures

Several have been mentioned following tuning=20
I remember Hitchin failing in this way.
How many times has this happened?
Please could I have any known details with dates to compile a list, or pe=
rhaps one exists.
These can be sudden catastrophic fails.
Has there been any injury or serious damage as a result?
