As some of you will know the above book was sold out soon after publication.
I am seriously considering an exact reprint with a comprehensive addenda and
additions bound in at the end of the book. I can only do this if I have
enough subscribers to cover most of the cost. I expect the cost to
pre-publication subscribers to be £35 (the same as the original). Of
