The only difference between this and Johnson's variaton of 
Middleton's is that you've replaced 2M 2W with FIMHW. 
That's very similar to a Tony Cox variation of Middleton's 
that has a MHWFI in place of 2M 2W.  (You can freely 
re-order MWH and FI because both produce part ends from 
Middleton's.)

I think I'd be inclined to describe it as "arranged from 
C. Middleton" without any further attribution.
