Surely it isn't possible to get a definitive answer to this, as the 
dating of virtually all bells of this date is only approximate. We 
have one in York of c.1350 (2nd at the former St John), and a dated 
exanple (tenor) in the same ring of 1408. Is this the earliest dated 
bell still in a ring?
