Depending upon exactly how you count them, since 2000 about fifty
times. What do I get for winning your bet? And, for what little it's
worth, I know of at least two further cases where bands intended to
extend methods (having deliberately designed them as a family to allow
ringing multiple peals at different stages) but failed to ring the
peal at the higher stage -- one such family was for your
father-in-law's seventieth birthday (they did actually sort of ring a
family, using the same name for an alliance major method and a cognate
little alliance royal method; in the end the peal of the extension of
the latter to max was not gone for).

Note also that the number of new methods extended to a higher stage
from a lower isn't the total number of times the rules on extension
are invoked: you can also go from a higher stage to a lower (see
Effingham Surprise, for example), and the rules can also be invoked to
*prevent* you from naming something as you might like.
