[r-t] [Re] Avon Extensions

I believe this extension of Avon was included in a Rod Pipe composition of
Spliced Sixteen which was attempted a couple of times in Birmingham in
about 2010, so it has been rung but not named.  I'm not sure how far any of
those attempts went though.
